Wholesale orders must have a copy of your relevant license or for Charities IRS Form 990 on file with us prior to sale.

Pricing information will be faxed or emailed after we receive your Wholesale Application and a copy either your Health Practitioner License, Resale License (or equivalent) or for Charities IRS Form 990.
